Job Description:

Join our compassionate team at a 152-bed skilled nursing facility in Willow Glen, San Jose, dedicated to providing exceptional post-acute and long-term care. We are seeking a dedicated Physical Therapist (PT) to conduct patient evaluations, develop personalized treatment plans, monitor progress, and educate patients and caregivers.
Responsibilities include hands-on therapy, ensuring regulatory compliance, and contributing to continuous improvement.
New graduates are welcome; previous skilled nursing or long-term care experience is a plus.
Qualifications include a Physical Therapy degree, valid licensure, and strong communication skills.
