Position Title: Support Coordinator

Department: Navigation and Quality

Reports to: Supervisor

General Description:

Provide coordination of services to a person with developmental disabilities through administrative support functions for the Case Management team.

Essential Duties/Responsibilities:

  • Provide a courteous, professional contact for the Navigation Services Team. Refer calls to appropriate agencies as needed. Follow HIPPA and TREs confidentiality policies in screening and triaging calls and email correspondence accurately and promptly. Communicate information about all Medicaid waiver services, including those not managed by TRE.
  • Work with TRE applicants to complete initial screening and BUS referrals and ensure completed applications are received.
  • Maintain databases such as BUS/CCMS/BILI for TRE applicants and participants.
  • Provide customer service to those applicants who initiate in-person contact with TRE to include completing applications, faxing consent forms, reviewing documentation, explaining services and programs, and referring individuals to community resources.
  • Attend resource meetings and collaborate with agencies on training opportunities in order to communicate and connect applicants with resources.
  • Provide excellent customer service through professional interactions and representation of TRE, prompt responses to email and phone calls, engaging supervisors when needed for difficult situations, having a positive and encouraging attitude about the work of the organization.
  • Maintain knowledge of Developmental Disability Determination, to include answering questions and referring to appropriate decision-making authority.
  • Complete waiting list contacts and ensure databases are updated (CCMS, BILI, BUS, etc.)
  • Coordinates transfers to and from TRE waitlists.
  • Conduct Colorado Choice Transitions (CCT) Quality of Life Surveys for individuals who are transitioning or have transitioned into the community.
  • Facilitate intake completion meetings, explaining TRE services and finding appropriate fit for individual. Follow-up with Long Term Care Professional Medical Information forms and conduct functional eligibility (ULTC 100.2) assessments.
  • Generate and distribute application packets and waitlist contact / change correspondence.
  • Send termination of services (803) notices according to regulation.
  • Create forms in user-friendly formats as needed or requested.
  • Maintain and assist with tracking and filing requirements for off-site filing and electronic filing systems.
  • Provide coverage for other positions within TRE as requested (after hours may apply on occasion).
    Other Duties/Responsibilities:
  • Completes a targeted case management (TCM) activity according to CMS and DDD Rules and Regulations as appropriate.
  • Remains knowledgeable and current of DDD Rules and Regulations, Program Quality Standards, expectations and best practices.
  • Participate in professional opportunities (workshops, conferences, readings, etc.) based on personal professional development plan.
  • Presence at worksite during customary work hours is essential.
  • Assist with special projects as requested.
  • Perform driving duties as related to the position.
  • Other duties as assigned.
    Job Qualifications:

    Knowledge, Skills and Abilities:

    Minimum: Three (3) years of experience as an administrative assistant, document control, customer relations, or community contact position. Intermediate to advanced knowledge and use of Microsoft Excel and Word and experience maintaining databases.

    Education and Experience Requirements:

    Minimum: A Bachelor's degree from an accredited college or university; or a two year degree plus two years of experience in the field of developmental disabilities; or five years of experience in the field of developmental disabilities (experience must be specifically documented and verifiable on your application.) If an individual does not meet the educational requirement, considerations may be made if the individual has experience working with long-term services and supports (LTSS) population, in a private or public agency, or has a combination of LTSS experience and education, demonstrating a strong emphasis in a human behavioral science field.

    Experience in an office environment including word processing, database management, spreadsheets, punctuation, grammar, and spelling; letter and report composition, office practices and procedures, record-keeping systems, office machines and their uses. Professional demeanor, ability to prioritize and accomplish multiple tasks, and ability to proactively negotiate difficult situations with tact and diplomacy. Must have accurate typing at minimally 60 words per minute. Possession of excellent oral and written communication skills is essential.

    Preferred: Working knowledge of Medicaid Waiver programs and requirements; experience working with persons with disabilities. Knowledge of and experience working with community resources and resource development. College level coursework in business administration, human services, or communications. Creativity and critical thinking are a plus.

    Material and Equipment Directly Used: Computers/tablets, Printers, Copy Machines, Personal Vehicle, Cell phones/telephone, Cloud based systems and software, etc.

    Working Environment/Physical Activities: Office, community, private residences, commercial properties, and virtual environment settings. Moderate physical activity required, such as walking, sitting, standing, and lifting up to 20 pounds.(Provides ADA accommodations at the request of the employee.)A working vehicle, valid CO drivers license, (unless active Military family) and minimum insurance are required. Insurance level of $100,000/$300,000/$100,000 liability coverage is recommended.
