Focuses on integrated care coordination to ensure physical health, behavioral health, and social services needs are assessed and provided to children and families. Collaborates with community partners and coalitions to share resources, leverage data, and promote trainings; advocates for system, policy, and programmatic change to support public health prevention efforts and to strengthen local and state capacity in meeting the needs of children with special health care needs and their families. This position is subject to grant funding. + Develops care plans for a specified caseload of children and families by assessing mental and physical health and social services needs, utilizing assessment tools from the state HCP program; involves family and community partners as appropriate. Develops, determines, and recommends care priorities and action steps; assigns responsibility for action steps to the appropriate party. + Monitors care plan action steps to ensure completion and adjusts or alters the care plan as needed; provides care coordination services for children and their families through home, clinic, and community visits; serves as liaison between health care and social service systems. + Advocates for children and families through implementing care plan with providers; ensures all recommended services are available to clients in an effort to ensure availability and accessibility of comprehensive health care services. Ensures care plan continuously reflects client needs and action steps are appropriately assigned and adjusted over time; coordinates and sets appointments with providers and attends appointments when necessary. + Mobilizes community partnerships to promote medical home and developmental screening for children. + Documents care coordination efforts in accordance with procedures; enters data into required databases. + Provides health education to families to improve health literacy. + Assesses the impact of local environmental, cultural, and social patterns on the public's health through collaboration with service providers and systems. Collects, compiles, and analyzes data to identify and address needs and service gaps for individuals, families, and communities. + May facilitate specific workgroups, partnerships, and collaborations to support and advance work. Works with key stakeholders and community partners to advocate for programs, systems, and policies that advance public health prevention efforts. + Plans programming for target populations using a variety of educational approaches to include small and large group presentations, a variety of facilitation methods, and development of health communication materials. + Develops, coordinates, and maintains relevant health and wellness program resources. Evaluates the effectiveness of programs and educational materials using a variety of evaluation tools. + Supports EPCPH priorities and programs within the agency through collaborative efforts. + Articulates the mission of El Paso County Public Health and develops partnerships to accomplish agency goals. + Participates in departmental emergency planning and response activities. + Promotes public health within the community. + Takes personal responsibility to provide exceptional customer service in order to promote and maintain a positive public health image and constructive working environment and fosters pride and professionalism in the workplace and community. + Performs other duties as required. Qualifications + Master of Social Work degree required. + Previous experience with care coordination, health promotion and coalitions, public health, child and youth development, and/or leading/facilitation of collaborative groups desired. + Must be proficient in Microsoft Office software, including Word, Excel, Outlook, and Teams. + Ability to communicate, motivate, and organize projects among a broad spectrum of personnel, frequently under deadline pressure. + Skill in demonstrating a cooperative, professional attitude to cultivate relationships both within and outside the organization. + Skill in prioritizing and organizing multiple tasks and the ability to complete projects in a timely manner. + Ability to respond in a flexible manner and reprioritize work as situations change. + Ability to demonstrate sound judgment by taking appropriate actions regarding questionable findings or concerns. + Ability to identify problems and work creatively to resolve them, considering the impact of actions on Public Health. + Ability to identify results of efforts and current or potential problems and develop and recommend corrective actions to supervisor. + Ability to establish and maintain effective working relationships with co-workers, clients, community partners and the general public. + Ability to write and maintain accurate records and reports to meet management objectives. + Ability to maintain the security of sensitive and confidential information. + Ability to work independently and in a team environment. + Skilled in training, presentation, and meeting facilitation. + Ability to perform under stress and when confronted with persons acting under stress. + Maintain regular and punctual attendance. + Must pass conditional post offer background investigation, physical examination, drug screen and motor vehicle record check. Licenses/Certificates + Must possess and maintain a valid Colorado Driver's License. Work Conditions Work environment may include potential exposure to infections, blood, and contagious diseases; potential exposure to pets, secondhand smoke and other environmental hazards. Travel will be required; work may be assigned in various locations. Will be required to work evenings and weekends as business needs dictate. Must be able to safely lift and carry up to 25 pounds and bend at waist or knees to get down to child's level.
