Job Duties--- Plans, implements, and evaluates patient care plansbased on patient assessment to optimize outcomes and maximize availableresources.--- Monitors, records, and communicates patient conditionas appropriate.--- Collaborates as needed across disciplines tocoordinate patient care, includingpatienttransfer, discharge, referral andspiritual/psychosocial support needs.--- Evaluates learning needs of patient and/or family andprovides patient/family education appropriate to age, culture, condition, andcircumstances. Effectively delegates patient care to ancillary personnel. Mayprecept student nurses and new hires.This position's pay is: $38.00/hour.
