Job Description Monday-Friday 8:00 am-5:00 pm Call Ratio- 1:5 Reason for need: Growth, looking to add 2 physicians to the group to make a total of 6 25-bed critical access hospital 15-20 patients seen per day Adult Medicine: 18 Provider would report to Practice Leader/SVP HR Not accepting Locums candidates Compensation and Benefits: $200K-$275K base wage depending on experience (RVU upside opportunity) Sign on bonus of $20K Up to $10K in relocation assistance Student Loan repayment and payout schedule: $20K per year for 2 years depending on contract length Retirement Program: 403B - 3% match ; 457B Health/Dental/Vision: Yes and family is covered - begins first of month after date of hire PTO/Holidays: 30 days CME days: 5 SUBMISSION REQUIREMENTS ALL OF THE FOLLOWING ARE REQUIRED AT NAME CLEAR UNLESS NOTED OTHERWISE.
