UCHealth is seeking a dedicated and compassionate Registered Nurse (RN) specialized in Acute Care Transition to join our dynamic team. In this role, you will be pivotal in managing patient transitions between acute care settings, ensuring a seamless and safe transfer process. This position requires a deep understanding of patient care, an ability to communicate effectively across various teams, and a commitment to uphold the highest standards of health care practices. Coordinate patient care with interdisciplinary teams across different care settings. Assess patient needs, develop, implement, and evaluate transition plans. Ensure compliance with all healthcare regulations and safety standards. Provide education and support to patients and their families regarding the care transition process. Monitor patient conditions and determine necessary care changes. Document patient care and transition details accurately and comprehensively. Strong clinical skills in a hospital setting Excellent communication and interpersonal skills Ability to manage multiple tasks in a fast-paced environment Knowledge of healthcare laws and regulations Proficiency in electronic health record (EHR) systems Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Registered Nurse (RN) license in the state of Colorado Basic Life Support (BLS) certification Advanced Cardiovascular Life Support (ACLS) certification preferred
