Schedule: Possible need for clinic coverage (7a - 3:30p).
Schedule will be mainly 24 hour call from 7a - 7a. Sporadic
coverage only. Patient Population: 100% Adult Caseload: 100% Adult.
Pediatric 0%. Neonatal 0%. Trauma 10%. Average of Patients per day:
15 Total Number of Beds: 300 Is the Locum Physician required to
supervise other healthcare providers?: NO Will other Vascular
Surgeons be present during the Locum Physician coverage?: NO
Vascular Lab Onsite: YES Number of Exam Rooms: 3 Level III Trauma
Describe office ancillary staff: Front office, 2 medical assistant,
1 mid level in the office, 2 mid levels at the hospital, 3 vascular
techs in the office EMR System: EPIC Credentialing Timeframe: 60-90
days
