This role requires strong organization, communication, and a passion for high-quality care. You'll work closely with patients, families, PCPs, therapy providers, and internal teams to drive optimal outcomes.Hybrid Schedule

• 60% in-person (minimal in-office time for meetings/training)
• 40% remoteKey ResponsibilitiesPatient Care & Coordination
  • Manage a diverse caseload with a focus on quality care and efficiency.
  • Provide oversight, coaching, and support to CNAs.
  • Develop and implement personalized care plans based on patient goals.
  • Coordinate with PCPs, therapy providers, and support teams for seamless care.
  • Educate and support patients and families on treatment and health management.Clinical Documentation
    • Maintain accurate, real-time electronic charting.
    • Ensure compliance with regulatory and client standards (24-hour turnaround).Communication & Collaboration
      • Build strong relationships with PCPs, therapy providers, and internal teams.
      • Escalate concerns to Clinical Leads as needed.
      • Participate in on-call rotation (-one weekend per month), holiday rotation, and possible weeknight phone triage.PTO & Benefits
