RN Refugee CoordinatorPeak Vista Community Health Centers is a

Compensation (Pay): $30.84 to $44.72/hourly based on
experience.

Job Summary: Maintains compliance for Peak Vista Community Health
Centers (PVCHC) contractual obligations to provide the domestic
medical screening to refugees, Humanitarian parolees, asylees and
other newly arrived persons to the Colorado Springs area referred
to PVCHC. Serves as point of contact for outside agencies regarding
all components of the domestic medical screening to include
referrals and follow-up.
Essential Duties and Responsibilities:
* Responsible for all data entry of patients seen at the clinic
thru the State Contract required on the State site and
Spreadsheets.
* Acts as liaison between patient, specialists, and PVCHC
providers.
* Performs all routine and emergent nursing functions pertinent to
efficient, quality, ambulatory patient care.
* Performs phone and face-to-face triage, appointment coordination
to include provider time and interpreter scheduling for health
screening, office visits, education and planning of treatments for
primary care-related illnesses and diseases and other infectious
diseases for all patients.
* Implements, evaluates and adjusts care of the patient after
comprehensive health history with regards to psychological,
socioeconomic, cultural and environmental factors, coordinating
referrals and follow up.
* Functions as back up to clinical team manager, covering clinic in
managerial role in the absence of clinical team manager to cover
staffing, patient complaints and all other contingencies as they
arise.
* When necessary this person will function as a float on the floor
to further assist Family Health and the needs of the team.
* The RN will follow the guidelines as in the Colorado nurse
practice act

Qualifications: To perform this job successfully, an individual

Education/Experience:
Education: Graduate of an accredited Registered Nursing Program
required.
Work Experience: Minimum of 3-years clinical nursing experience
required. Ambulatory care/Primary care setting experience
preferred. Triage experience preferred. Program Management
experience preferred.
Computer Skills: Intermediate - ability to access the
intra/internet to manage timecard, review policies and procedures,
and read company communications; use e-mail to communicate with
co-workers, leadership, and other departments; use and create a
variety of templates, complex tables, merges; manage table data,
sort and filter merges, and also perform basic work with existing
Macros; customize toolbars, import and insert graphs, embed Excel
data, and elaborate reports; work with multiple worksheets, filter
data, use integrate functions, and manipulate databases; customize
templates and the PowerPoint environment, and to make a
presentation interactive by using hyperlinks and action buttons
Certificates and Licenses: Current Colorado RN License. Current CPR
(BCLS) certification required.
