- 5+ years current RN or Social Work experience in an acute care,
palliative care, or hospice setting
- 2+ years experience of case Management
- 2+ years experience as a member of an interdisciplinary
team
- Knowledge of palliative and hospice regulations
- Knowledge of regulatory requirements necessary to integrate
into patient care delivery system
- Understand reimbursement for hospice and palliative care
services
- Full COVID-19 vaccination is an essential job function of this

- Master's degree in Nursing or Social Work-Current RN or
Licensed Clinical Social Work license
- National Certification in specialty area and/or Professional
Certification in Palliative care or related certification
- Ability to communicate effectively with patients and families
regarding goals, treatments and services available including
palliative and hospice careProof of immunity to vaccine-preventable

Colorado Residents Only: The salary range for Colorado residents is
$54,400 to $97,000. Pay is based on several factors including but
not limited to education, work experience, certifications, etc. In
addition to your salary, UnitedHealth Group offers benefits such
