The APC responsible for the coordination and communication of care of APC clinic patients throughout the course of treatment, including the outpatient clinic, the inpatient acute care hospital, ambulatory surgical setting and the post-discharge to home with home health care, or in a skilled- or long-term care facility, the APC Nurse Liaison will serve as the Hospital's inpatient and outpatient APC Educator, with attention to wound care of the limb salvage patient. This position is responsible for achievement of mutually agreed upon objectives at an expert level of practice. This person works with qualified healthcare providers to ensure the best outcome for the patient, working with the outpatient APC team, hospital case managers, discharge planners and other healthcare team members to coordinate, communicate and educate all of those involved in the care of the patient. Responsibilities include: tracking patients from outpatient to inpatient or other care facilities; communicating with nursing and other staff members to ensure that the clinical pathway is followed from start to finish; to report problems or concerns to the APC treating doctor; to utilize various forms of communication, including telemedicine, to discuss care plans and/or issues.Take your passion for enhancing the lives of patients and turn it into a career, join the RestorixHealth Team and APPLY TODAY!What We Offer:Monday - Friday schedule, no weekendsComprehensive benefits package (Medical, Dental, Vision, Life, 401k Match)Competitive compensationGreat work/life balanceWhat You'll Do:Is responsible for the successful implementation and ongoing management of APC clinical pathways in conjunction with standards, protocols, guidelines, policies and procedures.Available as a resource to nursing staff and ancillary departments to provide education on needs of patients with diabetic foot ulcers and/or other etiologies requiring amputation prevention servicesAssists in orientation and development of an APC treatment team as requiredRounds on APC inpatients, ensuring progress, proper wound care, and reports problems to the attending and APC doctorsRounds on APC patients in skilled or long-term care facilities to ensure clinical pathway compliance, proper wound care, and reports issues to the APC treating doctor.Meets with discharge planners to discuss disposition of APC patientsQuarterly, semiannually or annually, prepares analysis and Programs review report for analysis and evaluation of Programs' status and develops and implements action plan for changes/improvements as appropriate.Stays current on state-of-the-art approaches to diabetic/ischemic foot ulcer treatment.Assists with updates to Programs' policies and procedures.Tracks patients from start of care to discharge from program; keeps electronic records and prepares reports for the APC treating DPM or MD and the Program Director; makes recommendations and taking action as appropriate to insure continued success of the APC.Maintains relationships with hospital nursing staff, including case managers and discharge planners to ensure ongoing and vital communication from admission to discharge.Performs hands-on wound care in clinic or hospital when necessary but does not function as the wound care treatment nurseQuality Management:Monitors with the Clinical Manager and Medical Directors, accuracy and completeness of patient information entered into the medical record.Is an integral member of the quality management team, and participates in the Performance Improvement Program.Has responsibility for insuring that all key clinical indicators to include treatment completion and healing outcomes fall within established parameters.Monitors and ensures timely review of clinical charts by appropriate nursing and medical staff.Attends APC clinic meetings and reports on metrics related to the care coordination of the APC patients along the continuumWorks with assigned Quality Management APC team to implement recommendations and action plans resulting from such meetings.Finance:Understands and follows department budget guidelines and expectationsSeeks ways to enhance revenue and decrease expensesCommunity AwarenessAssists in development activities and expansion of services as necessary for success in the designated market.Plans in-service education lectures to partner skilled nursing, home health, and long term care nursing and medical staffDevelops, schedules and conducts educational and presentations to physicians and other potential referral sources as instructed by the Program DirectorEstablishes and manages effective phone inquiry response system, monitoring trends and addressing issues as appropriate.Utilizes telemedicine for education and outreach as appropriateMaintains relationships with key referral sources via routine visits and communication.Monitors trends in market area, providing educational presentations and in-services as needed and as opportunities arise while surveilling patients in SNFs or other post discharge settingsQualifications:Current RN LicenseMinimum two (2) years' experience working in acute care hospital settingWorking knowledge of various regulatory agencies, such as The Joint Commission preferred.Demonstrated leadership skillsDemonstrated teaching skillsDemonstrates a high degree of organizational skills.At RestorixHealth, we grow talent.
