Job Duties: Plans, implements, and evaluates patient careplans based on patient assessment to optimize outcomes and maximize availableresources. Monitors, records, and communicates patientcondition as appropriate. Collaborates as needed across disciplines tocoordinate patient care, including patient transfer, discharge, referral, andspiritual/psychosocial support needs. Evaluates the learning needs of the patientand/or family and provides patient/family education appropriate to age,culture, condition, and circumstances. Effectively delegates patient care toancillary personnel. May precept student nurses and new hires. This position''s pay rate is $38.00/hour.
