And we deliver these services through Empathy, Advocacy, and Integrity.Currently we are looking for a person with robust case management skills to join our team as a Care Manager. This position is part time (less than 35 hours per week), with potential to increase to full time. We are looking for someone who works well alone and in support of a team, is a self-starter who is adaptable, solutions-focused, detail-oriented, has a positive attitude and has a desire to contribute to an exceptional team environment.A Care Manager is the lead client facing position in our company. Care Managers oversee all the care management and care coordination needs of a client while also communicating with family and other constituents such as living communities, physicians, therapists, and financial and legal professionals. This includes, in part, assessing clients' living environments, finding appropriate placements, arranging, attending, and following medical care, working on insurance and benefits, coordinating services and advocating for our clients every step of the way. Our Care Managers may also oversee medication management for clients. In addition to Care Management responsibilities, our entire team pitches in to take on varying responsibilities to support the team and company.On call rotations and travel throughout the service area are requirements of this position.
